Credentia CNA Test Site: First Lady Permanente
Test Site Location: 901 Geer Road, Turlock, CA 95380

First Lady Permanente is an authorized CNA State Test site for Credentia testers in California. The test site fee is required to take the CNA CA State Test (Skills exam) at our Turlock Campus located at 901 Geer Road, Turlock. Once client has completed the registration with the agency (Credentia) they will then be required to submit this fee prior to test date or by the test date upon arrival. Forms of payment include: Website payment or in person- Cash or credit card

Required steps to test at our Turlock site:
Step 1: Register with CNA State Testing Agency (www.credentia.com)
Step 2: Submit CNA Skills Testing fee with Credentia on their website
Step 3: Study and prepare to take CNA skills test at our Turlock Site
Step 4: Pay Test Site Fee of $35.00 payable to First Lady Permanente prior to test through our website or pay cash on test date.

Items required to test:
- Government ID (Copies not accepted)
- Original social security card (Copies not accepted)
- Nursing Scrubs
- Signed waiver, provided by FLP test site
- Signed test site policies, provided by FLP test site
- Pay $35.00 test site fee on the FLP website or in person, fee is non-refundable and non-transferable

Authorized testers only:
The tester is the only authorized individual allowed to enter the test site building. Friends or family members are NOT authorized to enter the test site building for any reason. Restrooms are for testers only.

Credentia Test Results:
Login to Credentia platform to view testing results. Pass or fail is listed in your credentia user platform. The test site does not have this information available. Credentia customer service number 1-888-204-6249, Eastern standard time.

Test Site Fee:
First Lady Permanente located at 901 Geer Road, Turlock is an authorized Credentia Test Site for the California CNA Skills Exam. An additional fee to test at our Turlock site is required, this fee is $35.00 and may be paid in advanced through our website or in person.
Note: If tester does not make this payment prior to taking the skills exam they will not be authorized to test at our Turlock Campus.
